Certification of sickness
Use the form below to request a sick cert.
It is recommended that you request a sick cert at the time you are sick, not after. This will make it easier for your doctor to certify your illness.
Submitting the form does not guarantee a sick cert will be issued. We may need to arrange a phone consultation or an in-person consultation with your doctor to certify your illness (see fees here). In this instance, the cost of the certification will be included in the charge for the phone/in-person consultation.
The normal cost of certification is €20. This fee includes both a sick-note for your employer and a digital submission to the Department of Social Welfare. This is the fee for all private patients. Our reception team will send a digital link to your phone to pay the fee.
If you are a Medical Card or Doctor Visit Card patient, and you only require a Social Welfare Cert, there is no charge. However, there will be a charge of €20 for a sick-note for your employer.
How long will it take for my cert to be issued? The usual turnaround time for certificates is five working days. If we are certifying you for a long term illness we recommend you request a social welfare cert at least one week prior to the expiration of your current social welfare cert to avoid to avoid delays in social welfare payments.
